License Allocation Request Form

If you need to enable a user from another  SSL accounts to activate a license in your SSL account please fill out this form and send it to software.administration@teradyne.com for processing. This form is to be used to enable another party to activate licenses from your account. Use this for example when consigning license to a sub-contractor or loaning licenses between departments in your company.

License Ownership Transfer Request Form

If you need to make a permanent change in license owenrship between SSL accounts please fill out this form and send it to software.administration@teradyne.com for processing. This form is to be used when one entity is transferring ownership of a software or instrument performance license to another entity. 

Account Merge Request Form

If you would like to combine multiple SSL accounts into one please fill out the license allocation request form and send it to software.administration@teradyne.com for processing. Please note that the merge process will move all the entitlements and members to one account. This operation cannot be undone.

License Recovery Request Form

Please use this form to request recovery of licenses from a workstation or server that is no longer recoverable. If the machine is recoverable please follow normal re-host procedures. Please send the completed form  to software.administration@teradyne.com for processing. 

Note that all licenses will be returned to the SSL account and will need to be activated on the new host. If the existing host recovers in the future licenses will not be able to be rehosted from that machine.
